Job Title: Licensed Clinical Social Worker

We are seeking a highly skilled and compassionate Licensed Clinical Social Worker to join our team at Amergis. As a key member of our multi-disciplinary team, you will provide essential social work intervention and counseling services to patients and clients in a variety of settings.

Key Responsibilities:
  • Assess patient psychosocial needs and develop plans to meet established goals
  • Collaborate with healthcare professionals to plan and evaluate team goals for each patient
  • Provide counseling and crisis intervention services to individuals and families
  • Facilitate referrals to community agencies and provide financial counseling when necessary
  • Respond to crisis situations, including domestic violence and child abuse
  • Provide witness services and evidence in court cases
  • Visit schools to assess academic concerns and work with government agencies to help clients apply for Medicare and Medicaid
  • Arrange home visits to evaluate patient health, home environment, and family interaction/support
  • Assist patients and families in planning for present and future psychosocial needs
Requirements:
  • Master's degree in Social Work from a school of social work accredited by the Council on Social Work Education
  • Active license as an LCSW in the state of practice
  • Minimum of one year of social work experience preferred
  • Compliance with all relevant professional standards of practice
  • Participation and completion of Amergis' Competency program when applicable
