Overview: The Patient Care Coordinator will deliver specialized service management to clients, ensuring that comprehensive care is provided effectively. This role involves collaboration with various healthcare professionals, including Care Managers, Care Coordinators, and Transitional Care Managers, to guarantee that appropriate services are rendered for seamless care continuity.

Responsibilities:

  • Oversee the complete care strategy, integrating all services and plans pertinent to clients.
  • Conduct case management in accordance with Conflict Free Case Management regulations.
  • Engage in care management by reviewing client records, coordinating with Long-Term Services and Supports (LTSS) Care Managers through face-to-face/home visits, and managing schedules for these visits.
  • Visit clients in their homes, hospitals, assisted living facilities, or other care environments.
  • Travel is a significant component of this position, requiring up to 90% of time dedicated to conducting home visits within designated regions, with travel times potentially reaching 2.5 hours.
  • If documentation cannot be completed in the client's home, it will be done at a secure remote office or regional satellite.
