Responsibilities:
- Oversee the complete care strategy, integrating all services and plans pertinent to clients.
- Conduct case management in accordance with Conflict Free Case Management regulations.
- Engage in care management by reviewing client records, coordinating with Long-Term Services and Supports (LTSS) Care Managers through face-to-face/home visits, and managing schedules for these visits.
- Visit clients in their homes, hospitals, assisted living facilities, or other care environments.
- Travel is a significant component of this position, requiring up to 90% of time dedicated to conducting home visits within designated regions, with travel times potentially reaching 2.5 hours.
- If documentation cannot be completed in the client's home, it will be done at a secure remote office or regional satellite.
