Physical Therapist Home Health Specialist

We are seeking a skilled Physical Therapist to join our Home Health team. As a Physical Therapist, you will play a vital role in helping patients recover from injuries, illnesses, and functional disabilities, enabling them to regain their independence and live healthier lives.
Key Responsibilities- Plan and administer prescribed skilled physical therapy treatment and training for patients, aiming to achieve the highest level of physical function.
- Test and screen patients' physical strengths to assist physicians in evaluating their level of function and record findings to develop or pursue treatment programs.
- Develop and implement a conditioning/rehabilitation program consistent with the physician's Plan of Treatment and the overall goals of the patient/rehab team.
- Confer with physicians and clinical team members to obtain additional patient information and assist in developing, implementing, and revising the therapy treatment program and Plan of Treatment.
- Provide Physical Therapy Assistants and Home Health Aide staff with written instructions/care plans that reflect the current plan of care, supervise, and evaluate their performance.
- Accurately and promptly document patients' care observations, interventions, and evaluations, ensuring that interim (verbal) orders received from physicians and physical therapists are promptly and accurately documented, submitted for physician signature, and implemented.
- Report patients' progress to the patient's physician, Clinical Manager, staff, patient, and family, submitting evaluation, treatment plans, progress reports, and discharge summaries to the supervisor and care management staff.
- Degree from an accredited Physical Therapy Program (approved by the APTA)
- Minimum of one year physical therapy experience required
- Home Health experience preferred (OASIS Documentation a plus)
- HCHB (Home Care Home Base) EMR experience preferred
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Strong organizational and communication skills
