Physical Therapist Home Health Specialist

We are seeking a skilled Physical Therapist to join our Home Health team. As a Physical Therapist, you will play a vital role in helping patients recover from injuries, illnesses, and functional disabilities, enabling them to regain their independence and live healthier lives.
Key Responsibilities- Plan and administer prescribed skilled physical therapy treatment and training for patients, utilizing evidence-based practices and individualized care plans.
- Conduct thorough assessments to identify patients' physical strengths and limitations, informing the development of targeted treatment programs.
- Collaborate with physicians, clinical team members, and other healthcare professionals to ensure comprehensive care and effective communication.
- Supervise and evaluate the performance of Physical Therapy Assistants and Home Health Aides, ensuring high-quality care and adherence to established protocols.
- Accurately document patients' care observations, interventions, and evaluations, maintaining accurate and up-to-date records.
- Master's degree from an accredited Physical Therapy Program (approved by the APTA)
- Minimum of one year physical therapy experience, with Home Health experience preferred
- Current and unrestricted Physical Therapy license, as well as CPR certification
- Strong organizational and communication skills, with ability to work effectively in a fast-paced environment
