Geriatric Care Coordinator

Job Summary:
As a skilled Geriatric Care Coordinator, you will play a vital role in coordinating patient care and population-based management for patients in specifically defined geriatric or other patient populations. Your expertise will be utilized to plan and implement a comprehensive, multi-disciplinary approach to manage health conditions, utilization of resources and protocols, patient self-care, and implementation and evaluation of treatment plans across the care continuum.

Key Responsibilities:
  • Develop and implement individualized patient care plans in collaboration with physicians and healthcare teams.
  • Conduct thorough assessments of patient needs and develop strategies to address health disparities and promote health equity.
  • Collaborate with healthcare providers to ensure seamless transitions of care and coordinate services to meet patient needs.
  • Develop and maintain relationships with community resources and providers to ensure access to necessary services and support.
  • Monitor patient outcomes and make recommendations for alternative levels of care to ensure optimal health outcomes.
  • Stay up-to-date on relevant regulations, policies, and procedures to ensure compliance and quality care.

Requirements:
  • Master's degree in Social Work or related field.
  • Minimum 2 years of experience in case management or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work in a Labor/Management Partnership environment.

Preferred Qualifications:
  • Current and valid LCSW licensure.
  • Spanish language proficiency.
