Job Summary:

The WellBe Senior Medical team is seeking a skilled Advanced Practice Clinician to join our geriatric care team. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality, patient-centered care to our elderly patients in their homes.

Job Description:

Key Responsibilities:

  • Provide comprehensive geriatric care to patients in their homes, including initial assessments, ongoing care, and coordination with other healthcare providers.
  • Develop and implement individualized care plans that address the unique needs and goals of each patient.
  • Collaborate with our interdisciplinary team to ensure seamless care transitions and optimal patient outcomes.
  • Conduct home visits to assess patient needs, provide education, and support, and make recommendations for care.
  • Order and interpret diagnostic tests, prescribe medications, and manage patient care as needed.
  • Document patient interactions, assessments, and care plans in a timely and accurate manner.
  • Participate in regular team meetings to discuss patient progress, share knowledge, and address challenges.

Requirements:

  • Master's degree in Nurse Practitioner studies from an accredited program.
  • Active, unencumbered RN and NP licenses in the posted state.
  • Current board certification as an NP.
  • Experience in geriatrics, home care, or primary care preferred.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work independently and as part of a team.
