Join Our Team as a Geriatric Care Physician

We are seeking a skilled Physician to join our team at Fallon Community Health Plan. As a Geriatric Care Physician, you will be responsible for providing high-quality medical care to our elderly patients.

About the Role

This is a unique opportunity to work with a team of healthcare professionals who are dedicated to providing exceptional care to our seniors. As a Geriatric Care Physician, you will be responsible for:

  • Providing primary care to our elderly patients
  • Developing and implementing individualized care plans
  • Collaborating with our interdisciplinary team to ensure comprehensive care
  • Providing education and support to patients and their families
Requirements

To be successful in this role, you will need:

  • A medical degree and licensure to practice medicine in the Commonwealth of Massachusetts
  • Board certification in Internal Medicine, Geriatrics, or Family Practice
  • At least two years of experience working with a frail or elderly population
