Key Responsibilities- Service Coordination: Develop and implement individualized service plans to meet the unique needs of clients, ensuring access to a wide range of community resources and services.
- Case Management: Provide direct assistance in maintaining apartment living, including monthly inspections and liaison services between tenants, lease holders, and apartment complexes.
- Transportation: Arrange and provide necessary transportation to clients, promoting independence and community engagement.
- Assessment and Planning: Conduct thorough needs assessments and develop individualized case plans, taking into account the client's strengths, challenges, and goals.
- Record Keeping: Maintain accurate and detailed client records, ensuring compliance with agency policies and regulatory requirements.
- Supervision and Support: Collaborate with supervisors and other team members to ensure seamless service delivery and provide support to clients as needed.
- Education: Bachelor of Arts or Bachelor of Science in a human service-related field.
- Experience: Two years of experience working with individuals living with mental illness.
- License and Insurance: Valid Alabama driver's license and driving record acceptable to agency automobile insurance carrier, as well as a suitable vehicle for required travel.
- Training: Completion of the State of Alabama Adult Serious Mental Illness case management training.
- Knowledge: Mental illness, symptoms, and treatment regimens, as well as community resources and regulations governing human services provider agencies.
- Skills: Effective communication, interpersonal relationships, and documentation, with a strong ability to work autonomously and as part of a multidisciplinary team.
