Job Title: Telephonic Triage RN

At TriHealth, we are seeking a skilled and compassionate Telephonic Triage RN to join our team. As a Telephonic Triage RN, you will play a critical role in providing high-quality nursing care to patients and families through telephonic triage.

Job Summary:

The Telephonic Triage RN is responsible for assessing patient needs, providing guidance and support, and facilitating communication between patients, families, and healthcare providers. This role requires strong clinical judgment, effective communication skills, and a commitment to delivering exceptional patient care.

Key Responsibilities:
  • Conduct thorough telephonic assessments to identify patient needs and develop effective care plans
  • Provide education and support to patients and families on health-related topics and self-care
  • Facilitate communication between patients, families, and healthcare providers to ensure seamless care coordination
  • Collaborate with interdisciplinary teams to develop and implement patient care plans
  • Document patient interactions and care plans in a timely and accurate manner
Requirements:
  • Associate's Degree in Nursing
  • Registered Nurse licensure
  • 2-3 years of clinical nursing experience
  • Strong computer skills and ability to type and talk simultaneously
  • Knowledge of patient needs and healthcare systems
Working Conditions:

This role requires frequent sitting, talking, and typing. The ability to work in a fast-paced environment and prioritize multiple tasks is essential.
