Hospice Nurse Care Manager

As a Hospice Registered Nurse (RN) / RN Case Manager, you will play a critical role in our mission to provide comfort, peace, and dignity to those who need it most.
About the JobWe are seeking a highly skilled and compassionate RN to join our team as a Hospice RN / RN Case Manager. In this role, you will work closely with our interdisciplinary team to deliver high-quality patient care, manage patient records, and collaborate with healthcare professionals to ensure seamless transitions of care.
Responsibilities:
- Provide direct patient care services to hospice patients, including assessment, planning, implementation, and evaluation of care plans.
- Act as primary liaison between physician, patient/family, and hospice team, ensuring effective communication and coordination of care.
- Manage patient clinical records, including charting, reporting, and data entry.
- Collaborate with interdisciplinary team members, including LPNs, CNAs, and support staff, to deliver comprehensive patient care.
- Evaluate patient and family needs, assist in preparation and execution of plan of care for each patient, and maintain accurate and up-to-date clinical records.
Requirements:
- RN license in state of employment (or eligibility to obtain).
- 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology strongly preferred.
- Demonstrated flexibility and composure while supporting others in a time of crisis.
- Current driver's license and automobile insurance.
- Current CPR certification.
