Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse to join our care team at South Shore Rehabilitation & Skilled Care Center in Rockland. As a key member of our team, you will provide direct nursing care under the supervision of the RN to residents and supervise the day-to-day nursing activities performed by the nursing assistant to ensure the highest degree of quality care is maintained at all times.
Responsibilities:
- Meet the nursing and rehabilitation needs of each resident.
- Maintain current resident care plans on all residents under your care.
- Follow physician's orders and render complete nursing care, treatments, dressings, and other physician orders.
- Transcribe physician's orders promptly and patient records daily to assure accuracy and completeness.
- Document comprehensive and complete nursing notes.
- Visit each resident daily to evaluate resident needs.
- Maintain accurate awareness of residents on alert list.
- Document and report any unusual or significant findings; contact the physician when necessary.
- Administer medication according to procedure.
- Maintain an accurate and safe control of all drugs for which you are responsible.
- Initiate any and all emergency procedures as necessary.
- Demonstrate correct safety techniques.
- Respect and take care of facility equipment and not wasteful of supplies.
- Contribute to effective communication and pleasant working conditions.
- Contribute to and promote positive resident and family growth and expand job knowledge.
- Demonstrate leadership skills.
- Other duties assigned by manager.
Requirements:
- Must be a graduate of an approved LPN/LVN program.
- Must be CPR certified.
- Must possess a current, unencumbered license to practice as an LPN/LVN in this state.
- Minimum one year experience in a hospital, long-term care facility or other related health care facility is preferable.
