Case Manager

We are seeking a skilled Case Manager to join our team at Healthkeeperz, Inc. The successful candidate will provide advanced social work services for the Community Alternatives Program for Disabled Adults and Children, assisting in locating, coordinating, monitoring, and determining eligibility for social, medical, financial, and other services to meet the needs of our patients.
Key Responsibilities:- Assess and evaluate patient needs, identifying areas where social intervention and referral are necessary.
- Conduct initial pre-screening and assessment of patients and families, evaluating their needs and resources.
- Develop and implement social work plans of care to ensure the health, safety, and well-being of patients.
- Collaborate with healthcare professionals to ensure comprehensive patient care.
- Provide counseling and emotional support to patients and families, strengthening their support systems.
- Document patient progress and outcomes, ensuring accurate and timely reporting.
- Participate in interagency conferences and serve as a liaison between patients and provider agencies.
- Working knowledge of basic social work principles, techniques, and practices.
- Strong organizational, communication, listening, and assessment skills.
- Ability to travel to other office locations as needed.
- Bachelor's degree in social work or a related field from an accredited institution.
- One year of directly related community experience in the health or medical field, preferably in case management.
- Completion of an NC Medicaid-certified training program within 90 calendar days of employment.
This role requires sitting for extended periods, frequent stretching, talking, hearing, bending, and reaching. Occasional travel to patient residences may be required, and the ability to walk up stairs is necessary. The position may involve exposure to potentially hazardous situations.
