The LPN Medication Management Nurse is responsible for delivering nursing care to designated residents in accordance with established protocols and practices related to fundamental nursing, rehabilitative, and personal care services. This role emphasizes quality assurance, resident safety, and infection control as core responsibilities.
KEY RESPONSIBILITIES:
- Effectively communicates resident health concerns, changes, and incidents to nursing leadership and designated family members as required.
- Monitors residents for significant changes, adverse reactions, and any communicable diseases, ensuring that their needs are consistently met.
- Administers and documents prescribed medications, noting their effectiveness and any adverse reactions.
- Processes daily physician orders and reviews Medication Administration Records (MARs) regularly.
- Maintains an adequate supply of medications by reordering refills and new prescriptions as necessary.
- Assists residents with personal hygiene and comfort activities, including bathing and grooming.
- Prepares residents for meals and assists with feeding when required, ensuring a pleasant dining experience.
- Responds promptly to nurse call systems, ensuring residents remain clean, dry, and comfortable.
- Repositions bedfast residents at scheduled intervals and assists with skin care and treatments as assigned.
- Records vital signs and collects specimens as ordered, maintaining accurate documentation.
- Maintains cleanliness and orderliness of the nursing station and disposes of waste appropriately.
- Collaborates effectively with the care team, fostering positive relationships with residents, staff, and visitors.
- Completes incident reports promptly and takes necessary actions as required.
- Participates in the admission process for new residents and assists nursing leadership in ensuring smooth operations during shifts.
QUALIFICATIONS:
To excel in this role, candidates must possess the following:
- Completion of a state-approved Professional Nursing Program and current licensure as a Licensed Practical Nurse.
- Experience in Memory Care is preferred.
- Valid driver's license and the ability to work flexible hours, including evenings and weekends.
- Strong communication skills, with the ability to read, interpret, and write reports and correspondence.
- High level of interpersonal skills to manage sensitive situations and maintain confidentiality.
- Attention to detail to ensure accuracy in all tasks.
- Proficiency in English, both written and spoken.
- Willingness to travel locally and occasionally out of town.
