Responsibilities
  • Develop and implement personalized care plans for patients
  • Work collaboratively with patients, families, and healthcare providers to ensure smooth transitions of care
  • Provide education and support to patients and families on various topics, including pain management, skin assessment, and ulcer prevention
Requirements
  • RN Diploma degree or higher from an accredited school of nursing
  • Active and unencumbered Registered Nurse license in the state of California
