About the Role

Talent Software Services is seeking an experienced Nurse Care Coordinator to join our team. As a Nurse Care Coordinator, you will play a vital role in ensuring that patients receive the highest level of care possible.

You will be responsible for working closely with healthcare professionals to develop and implement patient care plans that meet the unique needs of each individual. This will involve conducting thorough reviews of patient cases to ensure quality of care and appropriateness of services provided.

The successful candidate will also have experience in evaluating healthcare resource utilization and identifying opportunities for improvement.

A salary of $90,000 - $120,000 per year is offered for this position, depending on experience.

Duties and Responsibilities
  • Develop and implement patient care plans that meet the unique needs of each individual
  • Conduct thorough reviews of patient cases to ensure quality of care and appropriateness of services provided
  • Evaluate healthcare resource utilization and identify opportunities for improvement
Qualifications
  • Active nursing license
  • Bachelor's degree in nursing or related field
  • Minimum of 2 years of clinical nursing experience
  • Strong knowledge of healthcare delivery systems and utilization management practices
