Job Summary:

This is a part-time (32 hours/week) position supporting patients in need of end-of-life care in Santa Rosa and surrounding areas.

Main Responsibilities:

  2. Patient/Family Care Needs Assessment: This involves identifying patient/family needs and implementing nursing interventions as necessary. Regular evaluation of results will inform adjustments to the Plan of Care.
  3. Plan of Care Development and Updates: The coordinator will work collaboratively with interdisciplinary team members to develop and update the patient/family Plan of Care throughout their journey.
  4. Care Service Cost Management: Demonstrated knowledge of costs related to patient care services and adherence to department guidelines are expected.
Key Responsibilities:
  1. Case Management: Manages an assigned caseload from admission through patient's death, providing comprehensive care coordination services.
  2. Patient/Family Education: Provides patient/family education on disease management, self-care techniques, and available community resources.
  3. Interdisciplinary Collaboration: Works closely with healthcare professionals, including physicians, nurses, social workers, and other disciplines to ensure seamless care transitions.
  4. Documentation: Accurately documents patient information, including medical history, treatment plans, and progress notes, adhering to hospital policies and procedures.
Requirements:
  • Bachelor's degree in Nursing or a related field
  • Active California Registered Nurse License
  • A minimum of 2 years of experience as a RN in a similar setting
