Role and Responsibilities of the Nurse Practitioner:
  • Deliver primary and preventive healthcare to long-term care patients in skilled nursing environments.
  • Assess and manage residents experiencing acute changes in their health status.
  • Engage in advanced care planning and chronic disease management.
  • Prescribe necessary medications, including controlled substances, in compliance with state regulations.
  • Order, evaluate, and follow up on treatments and diagnostic tests.
  • Maintain regular communication with nursing home staff regarding patient condition changes.
  • Collaborate effectively with primary care physicians.
Qualifications and Educational Requirements:
  • Valid and unrestricted Nurse Practitioner license in the state of Illinois.
  • Board Certification.
  • DEA licensure.
  • Clinical experience in adult, family, or geriatric specialties.
  • Experience in a long-term care environment.
  • Proficiency in using Electronic Medical Records (EMR).
  • Capability to manage patients with complex illnesses and multiple chronic conditions.
  • Ability to work within a multidisciplinary team to coordinate patient care.
  • Basic coding knowledge is preferred but not mandatory.
