Role Overview:
The Patient Care Coordinator is pivotal in enhancing patient management quality and satisfaction. This role promotes continuity of care and cost-effectiveness by integrating case management, utilization review, and discharge planning functions.
Key Responsibilities:
- Facilitates the integration of social services and case management into patient care, discharge, and home planning processes.
- Conducts thorough medical record reviews based on established indicators and criteria.
- Acts as a patient advocate by investigating adverse occurrences and providing staff education on resource utilization and discharge planning.
- Ensures effective utilization of clinical resources to achieve desired outcomes within specified timeframes.
- Evaluates patient satisfaction and quality of care from admission through discharge.
- Maintains regular communication with physicians to support optimal patient outcomes and cost management.
Additional Duties:
- Coordinates social services for patients and families to help them navigate the impact of illness.
- Completes comprehensive assessments of patient and family needs upon admission.
- Refers complex cases to social workers for further support.
- Enhances collaborative relationships to empower patients and families in decision-making.
- Facilitates interdisciplinary rounds to review treatment goals and post-hospital needs.
- Participates in developing patient care policies and protocols.
- Performs other duties as assigned by management.
Qualifications:
- Thorough understanding of case management principles and healthcare management.
- Strong communication and presentation skills.
- Ability to simplify medical terminology for patient understanding.
- Excellent organizational and time management abilities.
- Effective problem-solving skills with the capacity to multitask.
- Compassionate demeanor with the ability to work independently and collaboratively.
- Proficient in Microsoft Office and other relevant software.
- Willingness to travel locally or regionally as needed.
- Fluency in English, both spoken and written.
Education and Experience:
- High school diploma or equivalent required.
- Bachelor's degree in a healthcare-related field preferred.
- Valid RN licensure required.
- A minimum of 7 years of experience in utilization review or case management.
- Experience in discharge planning within a healthcare setting is preferred.
- Basic Life Support (BLS) certification required within the first 90 days of employment.
