- Develop and implement personalized discharge plans that meet patients' unique needs and preferences.
- Collaborate with healthcare teams to ensure timely and effective transitions to post-acute care settings.
- Provide education and support to patients and families to ensure a smooth transition and optimal outcomes.
- Associate's degree in Nursing or higher.
- Minimum two years of post-acute care experience or care management experience in a healthcare setting.
- Excellent communication and interpersonal skills with the ability to work effectively with patients, families, and healthcare teams.
- Knowledge of medical terminology and post-acute care referral processes.
- Registered Nurse licensure and Basic Life Support certification.
