We are seeking a highly skilled and experienced Acute Transitional Case Manager to join our team at Kaiser Permanente. As a key member of our Quality and Risk Management (QRM) department, you will play a critical role in coordinating care for identified members with complex medical conditions.
Key Responsibilities- Coordinate care for identified members with complex medical conditions in collaboration with hospital physicians, QRM staff, practitioners, and other providers.
- Develop and implement safe discharge plans for acute inpatient, skilled nursing, LTAC, and inpatient rehab facilities.
- Coordinate post-acute services and follow-up medical care to ensure continuity of care.
- Identify and communicate barriers to discharge plans.
- Ensure appointments and coordination of post-acute services with vendors.
- Minimum three (3) years acute hospital discharge planning or case management experience.
- Unrestricted current RN licensure in the state of Georgia required or documentation to show intent to seek license (or) LCSW (or) LMSW.
- Complex Case Management experience.
- Experience working with acute patient populations, including Medicare members.
- Functional knowledge of computers.
- Bachelor's Degree in Nursing or four (4) years of experience in a directly related field.
