Job Summary

HopeHealth Inc is seeking a highly skilled Intake Specialist to join our medical team. As an essential member of our team, you will act as the coordinator for the internal referral process, ensuring seamless communication and efficient patient care.

Key Responsibilities:

  • Navigate incoming referrals to departmental providers, ensuring timely and accurate processing.
  • Schedule new patients with the appropriate medical provider, ensuring the receipt of medical records as required.
  • Schedule outgoing referrals, including MRI and other required medical screenings and procedures.
  • Maintain up-to-date documentation through timely scanning and filing.
  • Verify patient demographics and update records as necessary.
  • Respond to patient phone calls and follow up on care plans to ensure compliance.
  • Communicate effectively with staff through documentation and maintain accurate records.
  • Participate in staff meetings and in-service training as required.

Requirements:

  • Excellent written and oral communication skills.
  • Strong interpersonal skills and ability to multitask.
  • Experience in a medical office setting.
  • Ability to work in a multi-disciplinary team and model healthy work practices.

Physical Requirements:

  • Adequate hearing to perform job duties in person and over the telephone.
  • Ability to communicate clearly to patients in person and over the telephone.
  • Visual acuity to perform job duties, including reading information from printed sources and computer screens.
  • Occasional lifting and carrying items weighing up to 10 pounds unassisted.
