Job Title: Outreach Specialist

Job Summary:

The Outreach Specialist will play a critical role in re-engaging individuals living with HIV (PLWH) into care and treatment services. This position requires a compassionate and strategic approach to locate and connect patients with Specialized Medical Case Management (SMCM) services.

Key Responsibilities:

  1. Conduct Outreach Activities: The Outreach Specialist will conduct outreach activities to locate patients who have fallen out of care and transition them to SMCM services.
  2. Client Interviews: Conduct interviews with clients to identify barriers to continuous engagement in care and treatment.
  3. Collaboration and Communication: Work closely with an inter-disciplinary team, including Medical Case Management and clinic staff, to ensure seamless client care.
  4. Client Support: Provide support to clients as needed, including informing them about available services and medical care benefits.
  5. Reporting and Documentation: Maintain accurate records and reports to track outreach progress and client engagement.
  6. Confidentiality: Ensure the confidentiality of Protected Health Information (PHI) in accordance with federal, state, and discipline-specific laws and regulations.

Requirements:

  • Education: Undergraduate degree with compassionate outreach experience.
  • Skills: Valid driver's license, good communication skills, creativity, flexibility, organizational skills, and knowledge of community resources.
  • Physical Requirements: Ability to travel, lift up to 10 pounds, and perform other physical demands of the job.

Work Environment:

The Outreach Specialist will work in a fast-paced environment, requiring flexibility and adaptability. The position involves frequent travel, both in-state and to national meetings, and requires direct client contact.
