Job Title: Client Care Representative

We are seeking a compassionate and detail-oriented Client Care Representative to join our team at Veterans Care Coordination. As a Client Care Representative, you will play a vital role in supporting our veteran and family clients by building trusting relationships, coordinating in-home services, and providing referrals to community resources and government benefits programs.

About the Role:

This is a hybrid role that requires a strong work ethic, excellent communication and interpersonal skills, and the ability to manage multiple tasks simultaneously. You will work closely with our veteran and family clients, as well as our home care agency, to ensure that our clients receive holistic support.

Responsibilities:
  • Build trusting relationships with veterans and their families, listening to their needs and providing support and resources as needed.
  • Coordinate and assist with a variety of in-home services, including transportation, personal care, and medical supplies.
  • Provide referrals to community resources and government benefits programs.
  • Maintain accurate client records and ensure all information is kept confidential.
  • Collaborate with the family and home care agency to ensure client receive holistic support.
Requirements:
  • Minimum 1 year of experience in customer service, social services, or a related field.
  • Strong communication and interpersonal skills, with the ability to build rapport with people from diverse backgrounds.
  • Excellent organizational skills and the ability to manage multiple tasks simultaneously.
  • Familiarity with veteran resources and benefits is a plus.
  • Proficiency in Microsoft Office Suite or Google Workspace is required.
  • Strong work ethic, a positive attitude, and a commitment to providing exceptional service.
