Hospice Nurse Practitioner Job Description

At Advanced Home Health, we are seeking a highly skilled and compassionate Hospice Nurse Practitioner to join our team. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals and families in need of hospice services.

Job Summary:

The Hospice Nurse Practitioner will be responsible for evaluating and assessing the health history, health promotion, diagnosing medical conditions, and managing health problems through the development of individualized plans of care. You will work closely with patients, families, and other healthcare practitioners to coordinate and collaborate on patient care.

Key Responsibilities:
  1. Provide comprehensive hospice services, including medical care, palliation, and management of terminal illness, in compliance with state Nurse Practice Act and under the supervision of a collaborating physician.
  2. Complete history and physical examinations, evaluate patient medical conditions and health history, order and interpret laboratory and diagnostic tests, diagnose health conditions, and document findings in patient medical records.
  3. Develop individualized plans of care, prescribe medications and/or treatments, obtain consultations or make referrals, and coordinate hospice care services.
  4. Complete hospice face-to-face visit requirements prior to third and subsequent hospice benefit periods to assess hospice eligibility based on hospice conditions of participation and local coverage determinations.
  5. Provide clinical information to the hospice medical director for interpretation of eligibility for hospice recertification.
  6. Review clinical information, coordinate and oversee patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners.
  7. Assure patients receive appropriate measures to control symptoms through collaboration with interdisciplinary team members.
  8. Review and develop protocols for treatment and propose options for interventions based on literature in collaboration with the collaborating physician.
Requirements:
  • Graduation from an accredited School of Nursing.
  • Completion of an accredited Nurse Practitioner Program that conforms with the Board's educational standards set forth in the California Code of Regulations Section 1484 or certification by a national organization/association whose standards are equivalent to those set forth in the California Code of Regulations Section 1484.
  • Current nursing licensure in State and CPR certification.
  • Master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience.
  • Accreditation from an approved certifying body for advanced practice nursing as required by State.
  • Certification in a specialist area preferred (e.g., Hospice and Palliative Nursing (CHPN), Pediatrics, or Geriatrics).
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
Skills and Abilities:
  • Excellent observation, verbal and written communication skills, problem-solving skills, mathematical skills, and nursing skills per competency checklist.
  • Prolonged or considerable walking or standing. Able to lift, position, and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, and/or crouching. Visual acuity and hearing to perform required nursing skills.
  • Ability to cope with the mental and emotional stress of the position.
Work Environment:

The Hospice Nurse Practitioner will work in patient homes as well as the agency office. The ability to drive in own car to and from patients' homes is required.
