Key Responsibilities- Work closely with the clinical care team to deliver high-quality patient care and optimize outcomes.
- Foster a collaborative environment that promotes open communication and teamwork among care providers, patients, and caregivers.
- Collaborate with other care team members to address gaps in care and promote efficient use of resources.
- Act as a patient advocate to ensure the highest quality of care and patient satisfaction.
- Perform routine medical procedures, including vaccine administration, blood draws, and specimen collection.
- Assist in patient education and provide support with related educational tools and resources.
- Respond to patient phone calls and maintain patient confidentiality in alignment with HIPAA regulations.
- Perform other duties as assigned to support the success of our primary care team.
- High School diploma or equivalent required.
- LPN License required.
- Preferred: LPN License/MA Certification.
