Job Summary:
We are seeking a qualified Care Manager to join our assisted living team. The successful candidate will be responsible for providing high-quality care and services to a consistent group of residents, while promoting their independence and dignity.
Main Responsibilities:
- Build Meaningful Relationships: Develop strong relationships with residents and their families through regular communication, support, and guidance.
- Provide Personalized Care: Assist residents with activities of daily living, such as bathing, dressing, and grooming, while promoting their physical and emotional well-being.
- Manage Resident Care Plans: Collaborate with the care team to develop and implement individualized care plans that meet the unique needs and preferences of each resident.
- Support Residency Team: Work closely with the residency team to ensure seamless transitions, efficient communication, and effective problem-solving.
Requirements:
- Education: High school diploma or equivalent required; associate's or bachelor's degree preferred.
- Certifications/Licenses: Current certification in CPR/First Aid; valid driver's license (if applicable).
- Experience: Minimum 1 year of experience working in long-term care, preferably in an assisted living setting.
- Skills: Excellent communication, interpersonal, and organizational skills; ability to work effectively in a fast-paced environment.
