Post Acute Transition Specialist

We are seeking a highly skilled Post Acute Transition Specialist to join our team at CarePartners. This role will act as a coordinator between internal and external referral sources and our Post-Acute Service Lines.
Key Responsibilities- Establish relationships with hospitals, skilled nursing facilities, assisted living facilities, ambulatory and inpatient care managers, physicians, payors, and other community referral sources.
- Provide education and promote collaboration to ensure appropriate patient placement through screenings and assessments.
- Work closely with Post-Acute providers, SNF's, and physician practices in the clinically integrated network (CIN) to ensure a responsive and smooth flowing referral and admission process.
- Collaborate with health system and community colleagues to insure quality driven, best practice across the continuum.
- Maximize positive financial outcomes for designated patient populations by facilitating appropriate Post-Acute placement and participating in Goals of Care/Advanced Care Planning dialogue.
- Bachelor of Science in Nursing or related field.
- Minimum of two years recent experience in a health-related field.
- Preferred experience: Eight years recent clinical experience in a relevant practice area.
- Speech Pathologist, Registered Nurse, Occupational Therapy, or Licensed Physical Therapy certification.
