Job Title: Physician Assistant - Acute Care

Capital Health Services is seeking a highly skilled Physician Assistant to join our team in Acute Care. As a Physician Assistant, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare team.

Job Summary:

The Physician Assistant will work closely with our interdisciplinary team to assess patients, develop care plans, and provide education to patients and families. You will be responsible for performing procedures, ordering tests, and consulting with other healthcare professionals as needed.

Responsibilities:
  • Assess patients to identify priority care needs and develop individualized care plans
  • Document patient assessment and teaching and learning needs
  • Obtain detailed nursing history and identify patients at physiologic and psychological risk
  • Consult with interdisciplinary team regarding patient assessment findings
  • Perform procedures necessary to implement plan of care based on care standards and protocols
  • Develop plan of care across the continuum and exercise discretion and judgment when revising based on physiologic and psychosocial findings
  • Order tests and consult as required by plan of care and review test results
  • Review patient plan of care with interdisciplinary team and discuss plan with patient and family
  • Document all patient care activities as per documentation standards
  • Evaluate outcomes of documentation and contribute to performance improvement initiatives through medical record review
  • Serve as a change agent by implementing practice initiatives
  • Coordinate patient care activities to maximize patient outcomes
  • Facilitate the assessment of patients learning needs and outcomes measures of patient education, access resources from all disciplines to achieve patient outcomes
  • Facilitate communication among departments to ensure continuity of care
  • Act as a primary resource to patient care team
  • Identify, provide, develop, and maintain learning resources for patient care team
  • Lead a quality improvement process
  • Recognize trends in patient populations and explore alternatives to care and assist in the revision of standards, guidelines, and protocols
  • Analyze practices that contribute to variance, draw conclusions from aggregate data, identify resources to assist in variance analysis, and assist with drawing conclusions from aggregate data
  • Assist in the development and revision of standards for patient populations incorporating a interdisciplinary approach and utilizing best practices and research findings
  • Develop and revise patient education programs and evaluate effectiveness of learning
  • Participate in clinical research and reviews current nursing research literature and demonstrates application to clinical practice
  • Recognize behaviors that impact the team function and implement strategies to optimize teamwork and serves as a mentor for team members
  • Seek leadership opportunities and facilitates leadership development among colleagues
  • Collaborate in the development and implementation of educational plan for identified needs
  • Reviews self development plan and professional goals and seeks new knowledge toward the progression of professional goals
  • Demonstrates evidence of self development by participating in educational programs
  • Provides educational programs to advance performance levels of staff
Requirements:
  • Graduate of an accredited physician assistant program, approved by the National Commission of Certification of Physician Assistants (NCCPA)
  • One-year experience working with primary care management or acute care preferred
  • NJ Physician Assistant License
