Position Overview:

We are seeking a Patient Care Manager to become an integral part of our team. In this role, you will report directly to the Executive Director, Administrator, or Senior Patient Care Manager. Your primary responsibility will be overseeing the daily operations of your designated hospice location, ensuring the delivery of high-quality clinical care to our patients.

  • Assume leadership of the site in the absence of the Executive Director, Administrator, or Senior/Executive Patient Care Manager.
  • Oversee clinical, administrative, and recordkeeping functions to maintain operational efficiency.
  • Respond promptly and compassionately to phone inquiries and referrals.
  • Participate in the on-call nursing schedule and provide direct, skilled nursing care in patients' homes.

Qualifications:

  • Must be a Registered Nurse with at least an Associate's Degree in Nursing from an accredited institution. State-specific requirements may apply.
  • A Bachelor's Degree in Nursing is preferred.
  • Minimum of three years of direct patient care experience in a clinical environment is required.
  • At least one year of management or supervisory experience is necessary.
  • Two or more years of experience in hospice or home care settings are essential.
  • Current nursing license in the state of employment is mandatory.

Licenses and Certifications:

  • Active nursing license in the state of employment.
  • CPR Certification is required.
  • Valid driver's license and current automobile insurance are necessary.

  • Ability to adapt to various tasks and maintain composure in a dynamic environment.
  • Effective communication skills across diverse socioeconomic backgrounds.

  • Exhibit professionalism, maturity, and cooperation in all workplace interactions.

Knowledge and Skills:

  • In-depth understanding of hospice care principles and the roles of various disciplines in providing these services.
  • Excellent patient assessment capabilities and strong oral and written communication skills.
  • Thorough knowledge of managed care principles and regulatory guidelines, including Medicare and Medicaid.
