About the Role

Baptist Memorial Rehabilitation Hospital is a leading provider of comprehensive physical medicine and rehabilitation programs. As a Case Manager, you will play a vital role in delivering exceptional patient care and outcomes.

Key Responsibilities
  • Collaborate with interdisciplinary teams to develop and implement individualized treatment plans
  • Conduct thorough assessments and identify potential barriers to care and discharge
  • Communicate effectively with patients, families, and caregivers to ensure seamless transitions and optimal outcomes
  • Coordinate weekly patient care team conferences to refine treatment plans and achieve identified goals
  • Review patient records to identify potential comorbid conditions and communicate findings to the HIM team
Requirements
  • Current licensure as a Registered Nurse, Social Worker, or Healthcare professional (Respiratory Therapist, Physical Therapist, Speech Language Pathologist, or Occupational Therapist)
  • Certification in Case Management or Rehabilitation Nursing preferred
  • Minimum of 2 years experience in an inpatient setting, preferably in acute or rehabilitation hospitals
  • Effective communication skills in English, with additional languages a plus
  • Basic computer skills in Microsoft Office and other software
