About the Role

UofL Health is seeking a skilled Social Worker to join our Care Coordination Department. As a Social Worker, you will play a vital role in promoting optimal healthcare outcomes for patients of all ages.

Key Responsibilities

  • Assess, plan, coordinate, implement, monitor, educate, and evaluate care for patients in an acute care setting.
  • Interact with physicians, advanced providers, residents, medical students, nursing staff, and other healthcare professionals to ensure comprehensive patient care.
  • Provide psychosocial assessments, discharge planning, information and referral services, counseling, and coordination of services to meet patients' health-related needs.
  • Collaborate with community agencies and providers to ensure seamless care transitions.

Requirements

  • Current licensure in Kentucky as a LCSW or CSW.
  • One year of experience as a certified social worker.
  • Basic Life Support (BLS) accreditation by the American Heart Association (AHA).
