About the Role
UofL Health is seeking a skilled Social Worker to join our Care Coordination Department. As a Social Worker, you will play a vital role in promoting optimal healthcare outcomes for patients of all ages.
Key Responsibilities
- Assess, plan, coordinate, implement, monitor, educate, and evaluate care for patients in an acute care setting.
- Interact with physicians, advanced providers, residents, medical students, nursing staff, and other healthcare professionals to ensure comprehensive patient care.
- Provide psychosocial assessments, discharge planning, information and referral services, counseling, and coordination of services to meet patients' health-related needs.
- Collaborate with community agencies and providers to ensure seamless care transitions.
Requirements
- Current licensure in Kentucky as a LCSW or CSW.
- One year of experience as a certified social worker.
- Basic Life Support (BLS) accreditation by the American Heart Association (AHA).
