Job Summary

The Licensed Vocational Nurse (LVN) - LPN (Wellness) - Monday Afternoon Shift is a part-time position at The Terraces of Los Gatos - a HumanGood community. As a member of our care team, you will be responsible for providing high-quality nursing care to our residents while promoting a safe and healthy environment.

Key Responsibilities
  • Provide nursing care to residents in accordance with their individualized care plans and the community's policies and procedures.
  • Develop and implement care plans in collaboration with the care team and residents' families.
  • Conduct regular assessments of residents' physical and emotional needs and make recommendations for changes to their care plans as needed.
  • Collaborate with other members of the care team to ensure seamless delivery of care and services to residents.
  • Participate in ongoing education and training to stay current on best practices in nursing care and community policies and procedures.
Requirements
  • Current LVN/LPN license in the state of California.
  • Current CPR certification.
  • At least 1 year of prior nursing experience in a senior care, skilled nursing, post-acute, or sub-acute care setting.
