Job Summary:

The Patient Care Coordinator is a vital member of our team, responsible for modeling our company values and promoting our philosophy. This role requires strong communication and coordination skills to schedule patient services and work with staff.

Key Responsibilities:

  • Schedule patient services and document schedules in a timely manner.
  • Participate in coordinating care with management and patient interdisciplinary teams.
  • Provide effective communication to patients, staff members, and other healthcare professionals.
  • Respond to agency and patient needs in a professional and creative manner.
  • Perform on-call responsibilities as assigned.

Requirements:

  • High school diploma or GED required.
  • Experience with patient scheduling highly preferred.
  • Prior customer service experience preferred.

Skills:

  • Mathematical skills: ability to add, subtract, multiply, and divide in all units of measure.
  • Language skills: ability to read, analyze, and interpret general business periodicals and technical procedures.
  • Strong written and verbal communication skills.

Work Environment:

The demands of this role require regular standing, walking, and manual dexterity. Adaptations can be made to accommodate team members with disabilities. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus.
