Key Responsibilities:
  • Analyze and interpret medication orders from physicians, ensuring appropriateness of medications, dosages, and administration routes.
  • Work alongside physicians and healthcare teams to create, implement, and assess patient-specific medication plans.
  • Provide education to patients and their families regarding medication therapies, including potential side effects and interactions.
  • Assess the safety and effectiveness of medication therapies, making recommendations for adjustments as necessary.
  • Conduct drug utilization reviews and manage formulary to ensure proper medication use.
  • Confirm the accuracy of medication orders and dispensing processes to safeguard patient health.

  • Comply with all relevant regulations, policies, and procedures related to pharmaceutical care.

Qualifications:
  • Bachelor’s or Doctor of Pharmacy degree from an accredited institution.
  • Valid pharmacist license in the state of California.
  • At least 2 years of experience in a clinical pharmacy setting.
  • Strong clinical acumen and analytical skills.
  • Exceptional communication and teamwork abilities.
  • Capacity to work autonomously in a dynamic environment.
  • Proficiency with electronic health record systems.
