We are seeking a skilled Clinical Coordinator to join our team at PennState Health. This is an exciting opportunity to work in a fast-paced environment and make a meaningful impact on the lives of our patients.

About the Role

This full-time position will be primarily based in our interdisciplinary office at Hershey Medical Center, Monday through Friday. As a Clinical Coordinator, you will be responsible for coordinating the compassionate, efficient, and effective care for individuals with serious illnesses in our palliative care clinics.

You will collaborate with referring providers, an interdisciplinary palliative care team, and community resources to address patient and caregiver needs in the ambulatory setting. Your role will also involve working closely with our team to provide individualized, innovative, and specialized care to patients in the community.

Responsibilities
  • Coordinate the compassionate, efficient, and effective care for individuals with serious illnesses in our palliative care clinics.
  • Collaborate with referring providers, an interdisciplinary palliative care team, and community resources to address patient and caregiver needs in the ambulatory setting.
  • Provide individualized, innovative, and specialized care to patients in the community.
  • Work closely with our team to achieve shared goals and objectives.
Requirements
  • Bachelor's degree in nursing required.
  • Three (3) years of nursing experience required.
  • Currently licensed to practice as a Registered Nurse by state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact required.
