About the Role

We are seeking an experienced Clinical Care Manager to join our team at PennState Health.

As a key member of our care management team, you will be responsible for coordinating patient care and ensuring that patients receive the highest level of quality care. This includes working closely with healthcare providers, nurses, and other stakeholders to develop individualized care plans that meet each patient's unique needs.

Key Responsibilities:
  • Coordinate patient care by working with healthcare providers, nurses, and other stakeholders
  • Develop and implement individualized care plans that meet each patient's unique needs
  • Maintain accurate and up-to-date records of patient care and progress
  • Collaborate with multidisciplinary teams to ensure comprehensive care
About You

To be successful in this role, you will need to have a strong background in nursing and excellent communication skills. You should also be highly organized, detail-oriented, and able to work effectively in a fast-paced environment.

Requirements:
  • Degree in Nursing or related field
  • Minimum 2 years' experience in a similar role
  • Excellent communication and organizational skills
  • Ability to work independently and as part of a team
Benefits

We offer a competitive salary of $85,000 - $110,000 per year, depending on experience, plus a range of benefits including health insurance, retirement plan, paid time off, and more.
