Key Responsibilities:
1. Deliver clinical pharmacy support in multiple wards and clinics, including internal medicine, oncology, surgery, mental health, and emergency care.
2. Utilize advanced clinical skills to assess and manage medication therapies, ensuring optimal pharmacological treatment for patients.
3. Conduct thorough evaluations of patients upon admission and throughout their hospital stay to determine the appropriateness of their medication regimens.
Clinical Practice:
4. Develop, implement, and monitor comprehensive pharmaceutical care plans tailored to individual patient needs.
5. Actively participate in multidisciplinary rounds, providing drug information and recommendations to enhance patient care.
6. Review electronic medical records to ensure the appropriateness of drug therapies and monitor for potential adverse reactions and interactions.
Patient Interaction:
7. Engage with patients to gather medication histories and provide education on medication use, compliance, and potential side effects.
8. Collaborate with healthcare providers in the medication reconciliation process to promote safety and adherence to treatment protocols.
Consultative Role:

Qualifications:
Applicants must possess a Doctor of Pharmacy (Pharm.D.) degree from an accredited institution and hold a valid pharmacy license. Experience in a clinical pharmacy setting is preferred.

This position requires flexibility in scheduling, including potential weekend and holiday coverage.
