Job Summary

The Addictions Care Center of Albany is seeking a highly skilled and compassionate Addiction Counselor to join our team in the Jail Based Addiction Counselor position. This role is part of the SHIP (Sheriff's Homeless Improvement Project) and is located in a separate wing of the Albany County Jail.

About the Program

The SHIP Program provides a comprehensive range of services to homeless individuals, including a roof, food, access to basic services, and support to become successful members of society. As an Addiction Counselor, you will play a critical role in assisting individuals struggling with substance abuse disorders.

Responsibilities
  1. Provide counseling services to clients on understanding addiction and applying addiction counselor knowledge to practical treatment.
  2. Provide addiction education and relapse prevention information to clients through Interface meetings.
  3. Coordinate with medical, mental health, addictions, correctional staff, and relevant agencies regarding within and outside the facility.
  4. Maintain excellent record keeping, tracking all activity and reporting progress to partners.
  5. Complete Department of Health and other agency surveys, documents, and forms.
  6. Attend bi-weekly meetings, including case conferences with treatment providers, to streamline coordination of services and discharge planning.
  7. Prepare bio-psych-social, LOCADTR reports for inpatient residential recommendations and obtain approval from Central Management Unit of the Department of Social Services.
  8. Coordinate phone screenings and work with other staff assigned to clients in other case management capacities.
  9. Schedule appointments for clients to be seen by Catholic Charities and other providers.
Requirements
  1. Working knowledge of addiction with CASAC or CASAC-T credentials a plus.
  2. Peer Advocate with at least one year of experience a must.
  3. High school diploma or GED, some college; completed CASAC Credentialing training.
  4. Valid Driver's License.
