FeldCare Connects is a pioneering app-based referral network that connects patients with top-notch home healthcare professionals. As a Physical Therapist, you will play a vital role in delivering quality care to patients of all ages in the Waianae and surrounding areas.

About the Role

This is an exciting opportunity for an Independent Contractor to join our network and work on a flexible schedule. You will have the freedom to choose your preferred coverage areas and work at your own pace, with no minimum requirement. Our team provides administrative support, including assigning, communication, scheduling, care coordination, and quality assurance, so you can focus on what you do best - treating patients.

Key Responsibilities
  • Deliver one-on-one treatment to patients, observing, recording, and reporting their response to treatment and changes to their condition.
  • Develop and implement personalized plans of care, ensuring the best possible outcomes for patients.
  • Utilize our intuitive Electronic Medical Record system to document patient information, making it easy to track progress and make informed decisions.
  • Work collaboratively with our team to ensure seamless care coordination and quality assurance.
Requirements
  • Current Physical Therapist license issued by the state Board of Physical Therapy.
  • Completion of an accredited Physical Therapy program.
  • Reliable personal transportation and a smartphone.
  • Bilingual skills a plus.
