Position Overview

Director of Case Management

The Director of Case Management is responsible for providing comprehensive leadership and strategic direction for Case Management services within the organization. This role emphasizes the promotion and enhancement of Coordinated Care throughout the Care Continuum. The Director holds ultimate responsibility, authority, and accountability for the Case Management Department, ensuring that daily operations align with both short-term and long-term objectives.

Essential Qualifications:

  • Active Registered Nurse license required
  • Bachelor's Degree in Nursing is mandatory
  • Experience as a Director of Case Management in an Acute Care Hospital setting is essential
  • Certification in Case Management (CCM or ACM) is preferred

Key Responsibilities:

  • Oversee quality of care and patient satisfaction through the management of policies, procedures, and continuous improvement initiatives
  • Lead, develop, and retain a motivated and engaged workforce
  • Design and implement research, education, and training programs to enhance patient care and services
