Physical Therapist

ATI Physical Therapy is seeking a highly motivated and detail-oriented Physical Therapist to join our team as a Care Model Specialist. In this role, you will be responsible for understanding and integrating ATI's Care Model into standard practice, leveraging available tools and resources to guide and optimize care plans and patient outcomes/experience.
Key Responsibilities- Evaluation and Care Planning: Performs thorough evaluations and establishes goal-based plans of care to ensure optimal patient outcomes.
- Regular Objective Reassessment: Implements regular objective reassessment points to track progress towards goal achievement and/or adjusts goals based on patients' recovery trajectory.
- Care Plan Development: Establishes care plans aligned to ATI Best Practice guidelines to achieve predictable and positive outcomes.
- Caseload Management: Manages caseload effectively, across the spectrum of each patient's plan of care.
- Treatment Delegation: Effectively delegates treatment among the clinical care team.
- Performance Expectations: Meets or exceeds established performance expectations, including patient outcomes collection targets, scheduling practices, productivity, caseload management, and documentation compliance.
- Communication: Communicates effectively and regularly with referral sources and other relevant stakeholders in a patient's plan of care.
Assists with daily clinic operational needs and tasks as needed.
Qualifications- Education: Minimum Bachelor's of Physical Therapy from an accredited institution.
- Experience: No prior experience required as a licensed PT if graduation requirement is met.
- Preferred Qualifications: Knowledge, skills, and abilities that enable the candidate to establish themselves as a leader for the patient care team, design and structure treatment plans to enable best possible patient outcomes, and effectively delegate care amongst the care-team.
