Overview:

The Rehabilitation Specialist is responsible for assessing, treating, documenting, and coordinating care for individuals experiencing limitations in functional mobility due to various conditions such as injuries, developmental challenges, or health-related issues. This role is essential in assisting patients to alleviate pain and enhance their motor skills, balance, coordination, and overall physical fitness, enabling them to achieve independence in daily activities.


Key Responsibilities:
  • Conducts thorough evaluations of patients using diverse objective assessments, skilled observations, and comprehensive interviews, ensuring documentation aligns with regulatory standards.
  • Develops a detailed and individualized plan of care based on evaluation findings, incorporating measurable goals tailored to the patient's specific needs.
  • Engages in interdisciplinary meetings to discuss evaluation outcomes, integrate insights from various disciplines, and collaboratively formulate patient care plans.
  • Delivers targeted, evidence-based therapeutic interventions aimed at helping patients reach their optimal functional capacity.
  • Demonstrates expertise in operating therapeutic equipment necessary for performing job functions effectively.
  • Advises on preventative strategies and adaptive tools to maintain or enhance the functional abilities of long-term care patients in collaboration with nursing staff.
  • Continuously monitors and evaluates the effectiveness of therapy interventions, making necessary adjustments to treatment plans.
  • Accurately records and submits patient billing information in a timely manner.
  • Confirms patient demographics and insurance details at the beginning of each care episode.
  • Delegates tasks appropriately to support staff while ensuring compliance with state regulations and payer requirements.
  • Provides clinical mentorship and oversight for students in Physical Therapy and Physical Therapy Assistant programs.
  • Independently prioritizes and organizes work tasks in accordance with departmental policies and patient scheduling protocols.
  • Manages the inventory of therapeutic supplies and equipment, forecasting needs for budget considerations.
  • Maintains cleanliness and organization of the workspace, including therapeutic equipment and documentation areas.
  • Offers consultation and therapy services to external organizations as contracted.
  • Facilitates training sessions for support staff and community members as needed.
  • Assists in the development of marketing materials and promotion of rehabilitation services.
