Job Overview

Key Responsibilities:

  1. Maintain consistent and reliable attendance as a fundamental aspect of this role.
  2. Exhibit professional conduct that aligns with the culture, mission, and values of Hope Network Behavioral Health.
  3. Identify and respond to signs of abuse, neglect, or exploitation, implementing safety protocols to protect individuals in care.
  4. Effectively apply a Person-Centered Approach and Individualized Service Plans tailored to each individual.
  5. Utilize effective communication skills and person-first language to foster collaborative relationships with individuals and their support networks.
  6. Encourage holistic health and wellness, addressing physical, emotional, and social needs, including nutrition and medication management.
  7. Conduct assessments to address medical and behavioral needs, ensuring appropriate responses to various situations.
  8. Complete Medication Administration Training and demonstrate proficiency in administering medications accurately.
  9. Assist with intake and discharge processes, ensuring smooth transitions for individuals entering or leaving care.
  10. Implement crisis prevention and intervention techniques, documenting incidents and communicating with team members as necessary.
  11. Maintain accurate records of care and incidents, utilizing established documentation procedures.
  12. Facilitate access to recreational and psychoeducational activities, enhancing the quality of life for individuals served.
  13. Support individuals in navigating challenges related to their rights and advocate for their self-determination.
  14. Recognize the importance of family and social connections, assisting individuals in building and maintaining these relationships.
  15. Perform necessary tasks to ensure the efficient operation of the facility, including maintaining cleanliness and reporting maintenance needs.
  16. Ensure a safe working environment, adhering to safety standards and protocols.
  17. Participate in staff meetings, training sessions, and team-building activities as required.
  18. May be required to transport individuals to medical appointments, ensuring safe travel.
  19. Other responsibilities as assigned.

Qualifications:

  1. High school diploma or GED preferred.
  2. Valid driver's license preferred.
  3. Certification in CPR/First Aid is required.
  4. Availability to work scheduled shifts, including weekends.
  5. Ability to lift up to 50 lbs and perform physical tasks as needed.
  6. Demonstrated empathy and understanding towards individuals served.
  7. Attention to detail in documentation and medication administration.
  8. Critical thinking and problem-solving skills to address various issues.
  9. Ability to work collaboratively within a diverse team environment.

Experience:

  1. Basic computer skills are required.
