A home health speech-language pathologist (SLP) delivers personalized care to individuals in their residences, assisting them in overcoming challenges related to communication, language, swallowing, and cognitive functions.

They may also provide support for patients utilizing augmentative communication methods.

Key Responsibilities:
Home health speech-language pathologists (SLPs) focus on individualized patient care to help manage communication and language issues.

Specific duties include:
Assessments:

  • Evaluating patients' requirements, including medical, psychosocial, and environmental factors, alongside assessing their speech, language, and swallowing difficulties.
Treatment Planning:

  • Creating and executing tailored treatment strategies in collaboration with the patient and their healthcare provider. These strategies aim to address specific functional needs, such as enhancing pronunciation, fortifying speech muscles, and mastering correct speech.
Therapeutic Interventions:

  • Assisting patients with their speech, language, augmentative communication, and feeding needs. This encompasses conducting oral, pharyngeal, and motor breathing exercises, as well as instructing clients on sound production and voice improvement.
Goal Development:

  • Setting achievable, measurable objectives for patients and monitoring their progress.
Communication Enhancement:

  • Facilitating both verbal and nonverbal communication for patients, while addressing cognitive disorders that may hinder their communication abilities. This includes providing guidance to clients and their families on managing communication and swallowing challenges.
Team Collaboration:

  • Cooperating with other professionals involved in the patient's care, such as registered dietitians, medical social workers, physical therapists, occupational therapists, and homecare nurses. This role also involves liaising with physicians, educators, and specialists to ensure coordinated care.
Documentation:

  • Completing necessary paperwork and maintaining updated patient records. Documentation requirements may differ by agency, but every patient interaction is recorded.

Visit notes detail patient status, treatment, responses, educational efforts, assessment of the visit, and future care plans.

Qualifications:
Education:
  • Possess a master's or doctoral degree from an accredited program in communicative sciences and disorders, recognized by the American Speech-Language-Hearing Association's (ASHA) Council on Academic Accreditation in Audiology and Speech-Language Pathology (CAA) or a similar accrediting body.
Clinical Experience:
  • Have a minimum of one year of clinical experience in speech-language pathology, which may include completing 1,260 hours of supervised postgraduate clinical practice.
Licenses and Certifications:
  • Hold a valid certification to practice speech-language pathology and maintain a current CPR certification. Additional ASHA certification may also be required.
Other Requirements:
  • Possess a valid driver's license and have access to an insured vehicle.

Compensation: $42 - $55 hourly
