Taylor, United States Home Care Wellness LLC Full time

A home health speech-language pathologist (SLP) offers personalized care to patients in their residences, assisting them in overcoming challenges related to communication, language, swallowing, and cognitive disorders.

Additionally, they may provide support for patients utilizing augmentative communication methods.

Key Responsibilities

Home health speech-language pathologists deliver tailored therapy to individuals in their homes, focusing on various communication and language challenges.

Core Duties Include:
Evaluations

  • Conducting comprehensive assessments of patients' needs, which encompass medical, psychosocial, and environmental factors, alongside evaluating levels of speech, language, and swallowing difficulties.
Treatment Plans

  • Formulating and executing customized treatment strategies in collaboration with the patient and healthcare providers, targeting specific functional requirements such as enhancing pronunciation, fortifying speech muscles, and mastering correct speech.
Therapeutic Interventions

  • Assisting patients with speech, language, augmentative communication, and feeding challenges. This involves performing oral, pharyngeal, and motor breathing exercises, as well as instructing clients on sound production and voice improvement.
Goal Setting

  • Establishing clear, measurable objectives for patients and monitoring their progress toward these goals.
Communication Support

  • Aiding patients in both verbal and nonverbal communication, while addressing cognitive disorders that hinder their communicative abilities. They also provide guidance to clients and their families on managing communication and swallowing issues.
Collaboration

  • Partnering with other professionals in the patient's care team, including registered dietitians, medical social workers, physical therapists, occupational therapists, and homecare nurses. They also liaise with physicians, educators, and specialists to ensure coordinated care.
Documentation

  • Completing necessary documentation and updating patient records. While documentation requirements may differ by agency, each patient interaction is recorded, detailing patient status, treatment, responses, and future care plans.

Qualifications

Education

  • Possessing a master's or doctoral degree from an accredited institution in communicative sciences and disorders, recognized by the American Speech-Language-Hearing Association's (ASHA) Council on Academic Accreditation in Audiology and Speech-Language Pathology (CAA) or another authorized accrediting body.
Clinical Experience

  • Having a minimum of one year of clinical experience in speech-language pathology, which may include completing 1,260 hours of supervised postgraduate clinical practice.
  • Licenses and Certifications
  • Holding certification to practice speech-language pathology and maintaining a valid CPR certification. Current ASHA certification may also be required.
Other Requirements

  • Possessing a valid driver's license and having access to an insured vehicle.
