Job Summary

FamilyForward is a non-profit organization dedicated to providing comprehensive therapeutic and education services to support biological, foster, and adoptive families. As a Foster Care Case Manager, you will play a vital role in delivering these services and making a meaningful difference in the lives of vulnerable children and families.

Key Responsibilities

• Develop and implement individualized plans to meet the unique needs of each client
• Provide case management services, including connecting clients with community resources and facilitating meetings with biological parents, caregivers, and other stakeholders
• Transport clients to and from appointments, court hearings, and other events as needed
• Collaborate with the Family Support Team to ensure seamless coordination of services and support
• Maintain accurate and detailed records of client interactions and progress

Requirements

• Bachelor's degree in a related field
• Experience working in foster care and/or with at-risk youth and families
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Commitment to innovatively achieving best results for children and families
