Admission Nurse

We are seeking a skilled Admission/MDS Nurse to join our team at Carespring. As a key member of our healthcare team, you will be responsible for assessing new and existing patients to ensure their care needs are addressed and documented on the Minimum Data Set (MDS).
Responsibilities- Oversee the MDS process, following the RAI manual and working with multiple other MDS nurses to complete initial assessments for new admissions.
- Supervise the DON to ensure MDS nurses properly manage patient admissions, MDS assessment, and care planning completion.
- Complete required submissions weekly or as designated by the Director of Nursing, reviewing validation reports to ensure accuracy.
- Maintain the ability to obtain and review Quality Metrics (QM's), working with the Interdisciplinary Team (IDT) to initiate improvement projects and improve resident outcomes.
- Attend IDT Medicare and Medicaid meetings to collaborate on the MDS process.
- Develop and update comprehensive care plans and nurse aide care plans (Kardex) in conjunction with the MDS.
- Oversee non-nursing sections of MDS and CAA's to ensure accuracy.
- Coordinate, manage, and monitor care plans, identifying problems and needs of residents and goals.
- Monitor residents for change of status to determine need for significant change per RAI.
- Ensure supportive documentation is accurate according to MDS/RAI regulations.
- Current LPN or RN license for the state in which the facility is located.
- Flexibility with working schedule and ability to work independently.
- Understanding of the MDS process and willingness to continue learning and remaining current with MDS updates set by CMS.
