Responsibilities:
  • Provide Skilled Physical Therapy Services: You will provide comprehensive skilled services as ordered by the attending physician, including physical therapy evaluations, treatments, and education to patients and their families.
  • Develop and Implement Individualized Plans of Care: You will develop and implement individualized plans of care for each patient, taking into account their unique needs and goals.
  • Communicate Effectively: You will communicate effectively with patients, families, physicians, and other healthcare professionals to ensure seamless care coordination and optimal patient outcomes.
  • Document Accurately: You will maintain accurate and timely documentation of patient interactions, treatments, and progress, in accordance with agency protocol and Medicare/Federal guidelines.
  • Participate in Interdisciplinary Team Meetings: You will participate in interdisciplinary team meetings to discuss patient care plans, share knowledge, and collaborate with other healthcare professionals to achieve optimal patient outcomes.
  • Stay Up-to-Date with Continuing Education: You will stay current with the latest research, best practices, and industry developments through ongoing education and training.
Requirements:
  • Current Physical Therapy License: You must have a current Physical Therapy license in the state of Ohio.
  • Minimum of One Year of Experience: You must have a minimum of one year of experience as a Physical Therapist in an acute setting.
  • Home Care Experience: Home care experience is desired, but not required.
  • Excellent Communication Skills: You must have excellent communication skills, both written and verbal, to effectively interact with patients, families, and healthcare professionals.
  • Ability to Work Independently: You must be able to work independently, with minimal supervision, to provide high-quality patient care.
