Job Title: Home Health Physical Therapist

Alternate Solutions Health Network LLC is seeking a skilled Home Health Physical Therapist to join our team in Akron, Ohio.

Job Summary:

We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to patients in their homes. As a Home Health Physical Therapist, you will work closely with our interdisciplinary team to develop and implement individualized plans of care that promote patient independence and improve quality of life.

Responsibilities:
  • Conduct thorough assessments of patients' physical abilities and develop personalized plans of care to address their needs and goals.
  • Provide skilled physical therapy services to patients in their homes, including exercises, stretches, and other interventions to improve mobility and function.
  • Collaborate with other healthcare professionals, including physicians, nurses, and social workers, to ensure comprehensive care and coordination.
  • Document patient progress and outcomes in a timely and accurate manner, using electronic medical records and other tools as needed.
  • Communicate effectively with patients, families, and caregivers to educate them on treatment plans, progress, and goals.
  • Participate in ongoing education and training to stay current with best practices and evidence-based care.
Requirements:
  • Current licensure as a Physical Therapist in the state of Ohio.
  • Minimum of one year of experience as a Physical Therapist in a home health or similar setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to lift, bend, and move patients as needed, with or without assistance.
  • Valid driver's license and reliable transportation.
