Job Title: Physical Therapist Home Health

We are seeking a skilled Physical Therapist to join our team at Alternate Solutions Health Network LLC. As a Physical Therapist, you will play a vital role in providing high-quality care to patients in their homes.

About the Job

This is a full-time position, working Monday through Friday, 8:15 am to 5:00 pm. The Physical Therapist will be responsible for providing skilled physical therapy services to patients in their homes, as ordered by the attending physician. The ideal candidate will have a current license in the state of employment and a valid driver's license and auto insurance in their name as a driver.

Responsibilities
  • Provide skilled physical therapy services to patients in their homes, as ordered by the attending physician.
  • Observe, record, and report patient reactions, changes, and progress to the attending physician and any appropriate agency staff.
  • Perform OASIS assessments to develop an individualized plan of care and make adjustments as needs change.
  • Treat patients to relieve pain, develop or restore function, and maintain maximum performance using physical means, such as exercise, massage, heat, water, etc.
  • Help decrease re-hospitalizations by front-loading visits for high-risk patients and teaching the patient on preventative measures and making good decisions.
  • Counsel patients and families on the disease/injury and how to manage in all phases of physical therapy.
  • Instruct patients and family members on proper use of equipment, such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices.
  • Oversee and supervise therapy services of patients provided by PTAs and when appropriate, nurse assistants.
  • Communicate with PTAs and nurse aides on supervisory visit results.
  • Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
  • Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
  • Attend in-service training and mandatory agency meetings.
Requirements
  • Physical Therapist with a current license in the state of employment.
  • Valid driver's license and auto insurance in their name as a driver.
  • Capable of all physical demands.
